3. Warning and Assumption of Risk for Participation- must be done online
4. Emergency Card- once each sports season - must be done online
5. Concussion Guideline- once each season must be done online
Proof of Insurance Coverage: You will be required to provide the name of your insurance carrier and your policy number on the LCPS Sports Emergency Card. If you don’t already have insurance coverage, you may purchase coverage from any source you desire. There are resources that can be found on line or through local insurance agents.
Protect your child with student K&K Student Accident & Football Insurance. If you don’t have other insurance, tis student accident insurance is vital. If you have other insurance, student accident insurance can help with deductibles and copays.

ANY SPORTS PACKAGE THAT IS NOT FULLY COMPLETED WILL BE RETURNED FOR COMPLETION. COMPLETED PACKAGES MUST BE RE-SUBMITTED TO THE ATHLETIC DEPARTMENT BEFORE THE ATHLETE WILL BE ALLOWED TO PARTICIPATE IN TRYOUTS OR PRACTICES.
**Separate signed forms are required for each school year. Please complete forms after May 1, 2017 to be eligible for the upcoming school year through June 30, 2018**
